Job Summary
We are seeking a dedicated and compassionate Registered Nurse to join our Home Health and Hospice team. The ideal candidate will provide high-quality patient care in various settings in patient homes, Home Plus's, Assisted Living, and Nursing Home setting. This role requires a strong commitment to patient advocacy and the ability to work collaboratively with multidisciplinary teams to ensure optimal health outcomes for patients of all ages.
Duties
- Assess and monitor patient health status, including vital signs and medical history.
- Develop and implement individualized care plans based on patient needs.
- Administer medications and treatments as prescribed by physicians.
- Provide education and support to patients and their families regarding health conditions and care plans.
- Collaborate with healthcare professionals in pediatrics, pain management, hospital medicine, and other specialties.
- Perform specialized nursing procedures such as ostomy care and PICU support when necessary.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Participate in quality improvement initiatives to enhance patient care services.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home health and hospice is required
- Experience in nursing home care, outpatient settings, or inpatient facilities is preferred but not required.
- Strong knowledge of pain management techniques and hospital medicine practices.
- Excellent communication skills with the ability to educate patients effectively.
- Demonstrated ability to work well under pressure while maintaining a compassionate approach to patient care.
- Familiarity with electronic health records (EHR) systems is a plus.
- Commitment to ongoing professional development and continuing education in nursing practices.
