Job Summary
At ValueCare Rehab, we deliver high-quality rehabilitation services in the comfort of our patients’ homes, including private residences, independent living communities, assisted living facilities, and adult day care centers. We are seeking a compassionate, skilled, and motivated Physical Therapist to join our dedicated healthcare team.
In this essential role, you will evaluate patients, develop individualized treatment plans, and provide evidence-based therapeutic interventions designed to restore mobility, alleviate pain, enhance functional independence, and improve overall quality of life. You will work closely with patients, families, and interdisciplinary team members to ensure the highest standard of care and positive clinical outcomes.

Responsibilities
- Conduct comprehensive evaluations of patients’ physical conditions, including mobility, strength, flexibility, and pain levels
- Develop individualized treatment plans tailored to each patient’s needs and recovery goals
- Implement therapeutic exercises, manual therapy techniques, and other modalities to promote healing and functional improvement
- Educate patients and their families on injury prevention, proper body mechanics, and home exercise programs
- Monitor patient progress regularly and adjust treatment strategies accordingly for optimal outcomes
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care plans
- Maintain accurate documentation of assessments, treatments, progress notes, co-sign PTA notes, and discharge summaries in compliance with healthcare regulations
Skills
- Valid licensure as a licensed Physical Therapist in the relevant jurisdiction
- Strong knowledge of anatomy, physiology, and evidence-based rehabilitation techniques
- Excellent communication skills to effectively educate and motivate patients of all ages
- Ability to assess complex physical conditions and develop effective treatment strategies
- Proficiency in documenting patient information accurately using electronic health record systems
- Compassionate approach with a focus on patient-centered care and empathy
- Ability to work independently while collaborating seamlessly within a multidisciplinary team

Pay: $55.00 - $70.00 per hour
