As a Home Health Registered Nurse (RN), you will provide skilled nursing care to patients in the comfort and safety of their own homes. You’ll play an essential role in helping patients manage their health conditions, recover from illness or injury, prevent complications, and achieve greater independence at home.
Working closely with patients, caregivers, physicians, therapists, and the interdisciplinary home health team, you will assess patient needs, develop and implement individualized plans of care, provide skilled nursing interventions, and monitor patient progress throughout the episode of care.

Key Responsibilities
- Conduct comprehensive skilled nursing assessments of patients in their homes, including physical, functional, psychosocial, and environmental assessments.
- Develop, implement, and regularly update individualized plans of care based on the patient's condition, physician orders, and established goals.
- Provide skilled nursing interventions including medication management, disease management, wound care, catheter care, injections, and other treatments within the RN's scope of practice and physician orders.
- Monitor patients for changes in condition and identify potential complications or clinical concerns requiring intervention.
- Communicate significant changes in patient condition to the physician, clinical supervisor, and appropriate members of the interdisciplinary team.
- Perform and document comprehensive patient assessments and reassessments throughout the episode of care.
- Educate patients and caregivers regarding medications, disease processes, treatment plans, symptom management, safety precautions, and strategies for maintaining health at home.
- Provide education and support related to chronic disease management, including conditions such as diabetes, hypertension, heart disease, COPD, and other diagnoses commonly managed in home health.
- Provide skilled wound assessment and wound care according to physician orders and established clinical protocols.
- Assess medication effectiveness, identify potential medication-related concerns, and reinforce medication adherence and safety.
- Evaluate the patient's home environment for safety risks, fall risks, infection prevention concerns, and barriers to achieving care goals.
- Coordinate care with physicians, physical therapists, occupational therapists, speech-language pathologists, medical social workers, home health aides, and other members of the interdisciplinary team.
- Participate in care coordination, case conferences, and other interdisciplinary activities as appropriate.
- Complete required Medicare, OASIS, and home health documentation accurately and within established agency timelines.
- Maintain accurate and timely documentation of assessments, interventions, patient responses, care coordination, and progress in the electronic medical record (EMR).
- Assist patients and caregivers in understanding when to contact their physician or seek emergency medical attention.
- Support appropriate discharge planning and help patients transition safely from skilled home health services when goals have been met or services are no longer medically necessary.
- Provide compassionate, professional care while respecting each patient's preferences, cultural background, privacy, and dignity within their home.
Qualifications
- Current, unrestricted California Registered Nurse (RN) license required.
- Graduate of an accredited nursing program.
- Minimum 1 year of professional nursing experience preferred; home health experience strongly preferred.
- Current CPR/BLS certification.
- Experience with Medicare home health, OASIS, and home health documentation preferred.
- Strong clinical assessment, critical-thinking, organization, and time-management skills.
- Excellent communication skills and the ability to effectively educate patients and caregivers.
- Ability to recognize changes in patient condition and respond appropriately within the RN's scope of practice.
- Ability to work independently while effectively collaborating with an interdisciplinary home health team.
- Familiarity with electronic medical records (EMR) systems preferred.
- Valid California driver's license, reliable transportation, and appropriate automobile insurance required for travel to patient homes throughout San Diego County.
