The Services Coordinator - II plays a vital role in delivering person-centered support to seniors and individuals with behavioral health needs. This energetic position requires a proactive approach to coordinating services, managing crises, and fostering independence among clients. You will work closely with multidisciplinary teams to ensure clients receive the appropriate resources, including behavioral health counseling, childcare assistance, and workforce development programs. Your leadership will empower clients to achieve their goals while maintaining safety and dignity. Duties
- Coordinate and monitor individualized service plans for clients, ensuring timely delivery of support services such as addiction counseling, child welfare assistance, and social work interventions
- Provide crisis intervention and crisis management, responding swiftly to urgent situations with sensitivity and professionalism
- Conduct assessments using motivational interviewing techniques to motivate clients toward positive change and engagement in their care plans
- Supervise and mentor junior staff or volunteers involved in client support activities, fostering a collaborative team environment
- Facilitate access to Section 8 housing programs, Medicare services, and workforce development resources tailored to client needs
- Maintain detailed documentation of client interactions, service progress, and incident reports in compliance with organizational policies
- Collaborate with community partners and service providers to expand resource availability and improve client outcomes
Experience
- Proven experience in behavioral health, social work, or related fields with a focus on crisis intervention and addiction counseling
- Supervising experience working with diverse populations including children, families, or individuals experiencing homelessness or disability
- Familiarity with child welfare systems, Section 8 housing programs, Medicare regulations, and workforce development initiatives
- Strong skills in crisis management, motivational interviewing, and behavioral health assessments
- Ability to handle sensitive situations with professionalism while maintaining a positive attitude
- Excellent communication skills for engaging clients effectively and collaborating with multidisciplinary teams
