Registered Nurse Case Manager
Mi-Health is seeking a Registered Nurse (RN) Case Manager to join our care team in Hawaii. This role is responsible for coordinating and overseeing the delivery of patient-centered care across the interdisciplinary team, ensuring that each patient’s plan of care aligns with their clinical needs, goals, and hospice eligibility requirements. The RN Case Manager conducts comprehensive patient assessments, develops and updates individualized care plans, and serves as the primary point of coordination between patients, families, physicians, and the care team.
Key responsibilities include managing an assigned caseload of patients, performing routine visits and assessments, symptom management and end-of-life care planning, documenting care in compliance with regulatory and payer requirements, and collaborating with social workers, chaplains, and other team members to support the physical, emotional, and spiritual needs of patients and families.
Minimum Qualifications:
- Active, unencumbered Registered Nurse licensed in the State of Hawaii
- 2–3 years of clinical nursing experience required; hospice, palliative care, home health, or case management experience strongly preferred
- Strong assessment, documentation, and care planning skills
- Ability to work independently in the field while coordinating closely with an interdisciplinary team
- Valid driver’s license and reliable transportation for patient home visits
Pay: From $48.00 per hour
