Foothill Acres Rehabilitation & Nursing Center, a Sub-Acute and Long-Term Care facility is seeking for a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating patients' needs and developing personalized treatment plans to help them achieve their rehabilitation goals. This role requires understanding of anatomy, physiology, and medical terminology, as well as experience working with geriatrics, and individuals with disabilities. As a Certified Occupational Therapist, you will work closely with patients, families, and healthcare professionals to ensure that all medical needs are met with the highest level of care. Responsibilities
- Provide a comprehensive occupational therapy evaluation based on MD orders.
- Assess resident needs and create treatment plans to improve functional abilities for daily living, such as dressing, grooming, and mobility.
- Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines.
- Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
- Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
- Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering top precautions.
- Document patient’s daily participation and progress on progress note accurately and timely.
- Document monthly updated treatment plan on recertification comprehensively and in a timely manner.
- Document patient’s discharge on a discharge summary as indicated.
- Document any caregiver or discharge in-services on an in-service form as indicated.
- Complete any and all other documentation forms required.
- Document care plans and information needed for the MDS as deemed necessary by the facility.
- Attend facility and rehab meetings as indicated by Supervisor and/or Regional Manager.
- Participate in facility committee meetings and act as a rehab delegate to the interdisciplinary team.
- Facilitate discharge planning by assessing readiness for discharge, recommending home safety measures, and providing training for patients and families
Qualifications
- Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy that is recognized by the AOTA.
- Current state licensure or eligibility for licensure as an Occupational Therapist.
- Experience in acute care settings or skilled nursing facilities is preferred.
- Ability to create engaging care plans tailored to individual patient needs.
Physical Demands
Employee should be able to lift, push and pull, transfer patients, lift weights, transport patients and perform treatment on different settings. Employee should be knowledgeable in and exercise good body mechanics during patient treatment.
Pay: $58.00 - $60.00 per hour
