- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe their health status to promptly identify any changes or concerns
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Utilize specialized equipment such as Hoyer lifts or wheelchairs to safely transfer clients with mobility challenges
- Maintain detailed documentation of patient care activities, observations, and progress in compliance with healthcare standards
- Support clients with memory care needs, including Alzheimer’s and dementia care, ensuring a safe environment and engaging activities
Qualifications
- Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation, vital signs monitoring, medication administration, and care planning
- Certifications such as CPR, First Aid, and HIPAA compliance are required or highly desirable
- Experience working with individuals with developmental disabilities, autism spectrum disorder, or dementia enhances your ability to deliver personalized care
- Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts
- Strong communication skills for working effectively with clients, families, and healthcare teams
- Compassionate attitude with a commitment to providing respectful and dignified care in home or community settings
