PDHS is seeking a Clinical Maternal Health Manager (CMHM) to ensure continuity of care to pregnant and postpartum women, men, infants and families. The primary role of this position is to assist in the design of care plans and care coordination, develop in-service staff training, and advise on participant and community education to address improving perinatal outcomes towards healthy birth outcomes. The CMHMalso, participates in and provides collaborative input in all QA/QI activities regarding overall care, benchmarks of maternal health, work plans, and staff enrichment activities. Furthermore, abides by local, state, and federal laws governing health and service information by maintaining confidentiality, security, and integrity of protected health information. The position is grant funded.

Education Requirement: Master’s degree in Social Work, Nursing, or a related field with at least three (3) years of experience. Maternal, Child and Family Health preferably having worked directly or collaboratively in a hospital, clinic, or community-based setting. Possess a state approved license.
Duties and Responsibilities:
- Facilitates the integration of care coordination services to pregnant, postpartum women, men as partners, infants, and family members
- Offer treatment planning, follow up, and tracking of various health issues related to pregnancy outcomes, mental health (i.e. depression, anxiety, mood disorder) and overall health
- Coordinates with the clinical and program team to develop and implement quality assurance and quality improvement plans to achieve success health outcomes for program participant’s health
- Participates in data collection, analysis, and tracking of program outcomes and support a team effort in developing theimplementation of corrective measures for performance measures if deficiencies arise
- Coordinate and facilitate staff training, community education and stakeholder workshops on topics relevant to social determinates of health that impact infant and maternal health
- Maintains current knowledge on emerging models to address culturally competent service delivery
- Build relationships with stakeholders in maternal care, including local experts, representatives of clinical and behavioral health organizations spanning professional societies, advocacy groups, health plans, research institutions and government entities including the BOI, MMR Committee, etc.
Required Knowledge and Skills: Must have a thorough knowledge of service delivery systems; ability to interact effectively on an individual and group-based level, coordinate activities with individuals, families, and groups, and manage one’s time effectively, follow up and track service delivery activities. Must be able to develop working relationships with individuals, agencies, organizations, and groups in a collaborative way. Must communicate effectively, both written and verbally. Be proficient in the use of computers and related software and data input systems. Additionally, you must have a clean driving record with reliable transportation. You should be able to lift up to 35 pounds.
