This isn’t urgent care or a 16-hour grind — it’s relationship-based primary care for Shasta County seniors, with administrative time paid on top of clinical hours and a care team that has your back.

Your panel is roughly half the size of a standard practice, your visits are unrushed, and a full care team (experienced nurses, MAs, and dedicated coordinators) handles the coordination through our Juniper Care Circle™. Compensation: $200,000 – $300,000+
- Guaranteed Annual Base: $153,600+ at full-time. Your floor, paid year-round.
- Paid Administrative Time: Charting and admin are compensated on top of clinical pay, every pay period — not absorbed into your day.
- wRVU Production Bonus: $14/wRVU. With CCM, APCM, and RPM built into your panel, production stacks every month up to $80,000+ annual upside at panel maturity.
- Quality & Access Bonus: Up to $20,000/year, with guaranteed minimums at every milestone (1-year anniversary and every 6 months after).
Base, paid admin time, and the Quality & Access Bonus are guaranteed cash. wRVU production adds upside on top.
The Role
- Run your own panel — significantly smaller than most primary care practices, designed for depth, not volume.
- Longitudinal primary care: chronic disease management (diabetes, hypertension, arthritis), Annual Wellness Visits, preventive care, and panel stewardship.
- Real APP autonomy with real physician backup — support when you want it, no one looking over your shoulder.
- AI-assisted charting plus built-in CCM, APCM, and RPM infrastructure — production credit for coordination you’re already doing.
- 4 clinical days per week (M/T/W/Th typical).
Schedule & Benefits
- Schedule: 4 clinical days a week with start and end times built around your life. Three-day weekends, every week.
- PTO: Generous bank with a voluntary annual cash-out option.
- Healthcare Stipend: $18,000/year — medical, dental, vision, wellness, your call.
- 401(k): 100% employer match up to 6% of eligible comp.
- CME: $2,000/year allowance.
- Malpractice + Tail: Employer-paid in full.
- Licensing: CA license, NCCPA/ANCC, and DEA fees fully employer-paid.
- Additional Shift Rate: $100/hour for hours beyond your regular schedule.
- Relocation Support: Available. We’ll help you get to Redding.
Who We’re Looking For
- PA (NCCPA-certified) or NP (ANCC/AANP-certified) with an active CA license or eligibility.
- Active DEA and eligibility for federal health care programs.
- Comfortable owning a longitudinal panel — and ready for the autonomy that comes with it.
