Overview
We are seeking a dedicated and experienced Travel Registered Nurse (RN) Case Manager to join our healthcare team in Duluth, GA. This role involves managing patient care coordination and case management services within a hospital setting, ensuring seamless patient transitions across various levels of care. Duties
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs.
- Develop individualized care plans that incorporate discharge planning, post-acute care, and home health considerations.
- Collaborate effectively with physicians, nurses, social workers, and other healthcare providers to coordinate patient care strategies.
- Manage utilization review processes to ensure appropriate hospital resource use while adhering to health regulation policies.
- Facilitate communication with patients and their families regarding treatment plans, progress updates, and discharge instructions.
- Maintain accurate electronic health records (EHR) and medical records in compliance with HIPAA regulations.
- Oversee diagnostic evaluations and coordinate outpatient or inpatient services as required for optimal patient outcomes.
- Engage in clinical case management activities across diverse settings including hospital medicine, emergency medicine, ICU, PICU, geriatrics, behavioral health, and hospice care.
- Participate in quality improvement initiatives related to medical-surgical care, infectious disease management, trauma services, and managed care programs.
- Ensure compliance with healthcare policies by staying informed about current health regulation standards and best practices.
Qualifications
- Valid nursing license with current registration; prior experience as a Registered Nurse is essential.
- Demonstrated critical care experience with exposure to Level I or Level II trauma centers preferred.
- Extensive ICU experience along with familiarity in emergency room settings and medical-surgical units.
- Proven background in hospital medicine, post-acute care, outpatient clinics, or home health environments.
- Knowledge of medical terminology, physiology principles, diagnostic evaluation techniques, and clinical case management procedures.
- Proficiency in electronic health records (EHR/EHR) systems and healthcare software applications.
- Strong understanding of HIPAA regulations and medical records management standards.
- Excellent communication skills for client interaction and interdisciplinary collaboration.
- Ability to assess complex patient conditions accurately and manage cases efficiently across diverse healthcare settings.
