Consolidated Tribal Health Project, Inc. is a rural community health clinic that was incorporated as a 501 ( c )(3) in 1984 to address the disparity in health care among Native American people living in Mendocino County.

Salary Range - $150,000-$170,000
POSITION: (FNP) Family Nurse Practitioner with Pediatric Experience DEPT: Medical
REPORTS TO: Medical Director FSLA STATUS: Exempt
SUPERVISES: None
POSITION SUMMARY : Provide direct patient care to patients seeking medical care at CTHP.
PRIMARY RESPONSIBILITIES :
- Under the direction of CTHP Medical Director provide direct patient care: ob/gyn; pediatrics; general adult
- primary care, acute and chronic.
- Provide direct services to pregnant women and their infants, including set-up and visits to satellite clinic sites. Services to include pregnancy tests, follow-up exams.
- Coordinate service with other department staff.
- Identify and coordinate outreach services and participate in set-up of outreach service network to include county-wide needs assessment of outreach services available to CTHP clients.
- Identify and coordinate countywide network of health care providers and support service professionals for perinatal services.
- Working with the Medical Director and other programs and/or agency staff to establish procedures with intrapartum care providers for transfer of patient services and relevant information.
- Provide and attend staff in-service training.
- Coordinate statistical reports required by IHS and other funding agencies.
QUALIFICATIONS & REQUIREMENTS :
- Current California license as a Family Practice Nurse Practitioner (FNP).
- Current CPR certificate3. Appreciation of Native American culture and understanding of economic needs of rural Native America community.
- Experience in community clinic setting; ability to provide quality medical care to low-income community
- Strong communication skills essential.
- Ability to work in a multi-disciplinary environment.

Compensation Package:
- Provider Recruitment Program/Bonus-$50,000 for nurse practitioners/physician assistants/certified nurse midwives (NPs/PAs/CNMs)
- 401K available, and 5% employer contribution at 90 days.
- Employer provides $50,000 life insurance policy for full-time employees and additional coverage is available for purchase.
- Relocation Costs
- CTHP can help facilitate the Indian Health Service (IHS) Loan Repayment Program (LRP). This program offers up to $50,000 in loan repayment for qualified healthcare professionals, in exchange for a two-year service commitment at facilities serving American Indian and Alaska Native communities. Additionally CTHP is a qualifying employer for the Public Service Loan Forgiveness (PSLF) program. As a participant in the PSLF program, you can have your remaining Direct Loans forgiven after making 120 qualifying monthly payments under a qualifying repayment plan while working full-time at our clinic.
