Our Lady of Providence is a Level III residential care home located on 2.5 acres in the heart of Winooski, VT. We provide comprehensive daily living and medical support, including hospice care. We are a small, warm, and collaborative community focused on dignity, kindness, and teamwork.
Position Overview
Under the guidance of the Director of Nursing Services (DNS), the LPN or RN nurse is responsible for resident care management, clinical oversight, and shift leadership in accordance with Vermont DAIL Level III regulations and facility policy. Nurses collaborate with providers and families, supervise care staff, and ensure accurate clinical documentation.
Average census: 40 residents. Each shift is staffed with one nurse, two medication assistants (except overnight), and three to four care assistants.
Key Responsibilities
- Provide and oversee resident care management
- Supervise and support care & medication assistants
- Communicate with physicians, hospice, families, and interdisciplinary partners
- Review, enter, and implement medical orders
- Ensure regulatory compliance and safe clinical practice
- Accurately document assessments, care, and orders in PointClickCare
- Serve as shift lead LPN or RN nurse and clinical resource for staff
Shift Options
- Days: 7:00 AM - 3:30 PM
- Evening: 3:00 PM - 11:30 PM
- Overnight: 11:00 PM – 07:30 AM
Committed Hours
- Full-time: 32–40 hrs/week
Weekends & Holidays
- Shared equitably
- Every other weekend required
- Option to split weekends with a teammate
Compensation & Differentials
- Base Pay: $35.00–$42.00/hr
- Evening: +$1.50/hr
- Overnight: +$2.00/hr
- Weekend: +$0.50/hr
- Incentive bonuses available when eligible
