The CHAMP / Care Team Support Specialist is a hybrid field-and-office position
designed for experienced caregivers who demonstrate exceptional reliability,
professionalism, and client rapport. This role supports both client care and agency
operations by responding to urgent staffing needs, ensuring quality standards, and
maintaining strong communication with clients, families, and the office team.
Key Responsibilities
Urgent Coverage & Call-Out Support – Respond quickly to caregiver call-outs
and provide temporary coverage – Maintain readiness to step into client homes
with professionalism and flexibility – Communicate schedule changes promptly
with the office
Client Reassessments & Care Plan Updates – Conduct in-home
reassessments and monitor changes in condition – Update care plans to reflect
current needs, preferences, and safety considerations – Collaborate with families
and office staff to ensure continuity of care
Supervisory Visits – Complete routine and as-needed supervisory visits –
Observe caregiver performance, provide coaching, and reinforce best practices –
Document visit findings and escalate concerns appropriately
Client Satisfaction & Quality Assurance Calls – Make regular satisfaction calls
to clients and families – Track feedback, identify trends, and support service
recovery when needed – Strengthen client relationships through proactive
communication
Office & Administrative Support – Assist with scheduling, documentation,
filing, and care coordination tasks – Support onboarding activities such as
caregiver shadowing or skills checks – Help maintain organized, accurate client
and caregiver records
Requirements
Minimum 2–3 years caregiving experience (CNA preferred)
Strong understanding of ADLs, dementia care, safety protocols, and home care
standards
Excellent communication skills with clients, families, and staff
Ability to adapt quickly to changing needs and environments
Reliable transportation and willingness to travel between client homes
Professional, compassionate, and solutionsfocused demeanor
Comfortable working with pets and in varied home environments
Pay: From $17.00 per hour
