Provide high-quality speech-language pathology services in accordance with professional standards and organizational policies and ensure comprehensive, patient-centered care through collaboration and evidence-based practice.
Implement standards of care for speech-language pathology services and maintain compliance with all terms outlined in the Service Agreement.
Perform thorough assessments of patients’ speech and language abilities, including home environment evaluations when appropriate, and determine equipment needs.
Identify functional speech and language deficits and create individualized treatment plans to improve communication and overall patient function.
Administer speech therapy programs using specialized techniques and equipment tailored to patient needs.
Evaluate and recommend communication aids or devices as necessary to support patient progress.
Work closely with referring physicians and interdisciplinary team members to ensure coordinated, comprehensive care aligned with the total plan of care.
Attend case conferences, in-service trainings, and other meetings to support integrated care delivery.
Prepare and submit clinical notes and reports in compliance with organizational policies and timelines.
Stay current with best practices, research, and clinical standards in speech-language pathology.
Educate patients, families, and caregivers on treatment plans, techniques, and strategies to promote continuity of care.
Identify additional patient needs and make referrals to other services when appropriate.
Provide guidance and supervision to home health aides for tasks directly related to speech-language therapy plans.
Perform additional responsibilities as required to ensure the effective delivery of speech-language pathology services.
Qualifications:
Master’s or Doctoral degree in Speech-Language Pathology (SLP).
Licensed as a Speech-Language Pathologist in the state of practice.
1 year of clinical experience in Speech-Language Pathology is preferred.
Strong verbal and written communication skills and excellent organizational abilities.
