Community Resource Associates (CRA) is seeking a compassionate, organized, and dependable Case Manager to provide case management services to individuals enrolled in Oklahoma’s ADvantage Waiver Program for the Tulsa and surrounding areas.
Our Case Managers play an important role in helping older adults and individuals with disabilities remain safe and independent in their homes and communities. This position combines member advocacy, care coordination, in-person home visits, telephone monitoring, service planning, and documentation.
Case Manager Role:
- Manage an assigned caseload of ADvantage Waiver members
- Conduct required in-person home visits and telephone monitoring
- Assess member needs and develop individualized, person-centered plans of care
- Coordinate services with members, families, healthcare professionals, providers, and other community resources
- Advocate for members and help address barriers that may affect their ability to remain safely in the community
- Monitor services to ensure members' needs are being met and identify changes that may require intervention
- Complete required documentation accurately and within program timelines
- Participate in interdisciplinary team meetings and communicate with providers regarding member needs and services
- Assist members in accessing appropriate community resources while encouraging independence and self-direction
- Maintain compliance with ADvantage Waiver policies, procedures, and documentation requirements
Position Details
- 1099 independent contractor position
- Minimum 6-week in-office training period required before transitioning to independent caseload management
- Primarily field/home-based following completion of training
- Case Managers must be available by phone Monday–Friday, 8:00 AM–5:00 PM
- Scheduling of member visits provides flexibility within program requirements and required timeframes
- Local travel is required for member home visits
- Ongoing support and guidance provided by experienced Case Management Supervisors and agency leadership
Minimum Qualifications
Applicants must meet at least ONE of the following ADvantage Waiver Case Manager qualification pathways along with at least 2 years of paid professional or technical experience providing direct care services to aging or disabled populations
- Bachelor's degree
- Registered Nurse (RN)/Licensed Practical Nurse
- Physical Therapist Assistant (PTA)/Certified Occupational Therapy Assistant (COTA)

- Is comfortable working independently while remaining accountable to program and agency requirements
- Has strong organizational and time-management skills
- Communicates professionally and compassionately with members, families, providers, and coworkers
- Is confident advocating for member needs and coordinating services
- Can manage deadlines and maintain thorough, accurate documentation
- Is comfortable using web-based documentation systems and technology
- Has reliable transportation and is able to travel to members' homes
- Values person-centered care and helping individuals remain safely and independently in their communities

Community Resource Associates provides Case Management and Home Care services through Oklahoma's ADvantage Waiver Program. Our Case Management team focuses on strong member advocacy and personalized support through both telephone contact and in-person visits.
