81 Oaks Senior Living is seeking a compassionate, skilled, and dementia-experienced Licensed Practical Nurse (LPN) to serve as a clinical leader within our Memory Care neighborhood.
This role combines hands-on clinical care, dementia-informed support, and team leadership, working closely with the Director of Clinical Care and Director of Memory Care to ensure residents receive exceptional care and quality of life.
The Memory Care LPN partners with caregivers, medication technicians, families, and leadership to monitor resident health, respond to changes in condition, and guide staff in providing safe, compassionate dementia care. This role assumes a clinical leadership presence on the floor to support the care team.
In addition to supporting the Memory Care neighborhood, this role may provide clinical support to the Assisted Living side of the community as needed.

Provide clinical oversight and nursing support within the Memory Care neighborhood

Collaborate closely with the Director of Clinical Care and Director of Memory Care on care coordination and clinical needs

Provide clinical support to the Assisted Living neighborhood as needed
Clinical Care & Resident Monitoring

Monitor residents’ health status and identify changes in condition.
-
Perform nursing assessments and respond appropriately to clinical concerns.
-
Administer medications within LPN scope according to physician orders.
-
Communicate with physicians, pharmacies, and providers regarding resident care needs.

Support medication management and assist with resolving medicationrelated concerns.
-
Assist with clinical followup related to hospitalizations, medical appointments, or new physician orders.
-
Apply dementiainformed care practices when supporting residents living with Alzheimer’s disease and related dementias.
-
Recognize behavioral and psychological symptoms of dementia and guide staff on appropriate responses.

Help staff understand how medical conditions, infections, pain, or medication changes may influence dementiarelated behaviors.

Assist in identifying nonpharmacological approaches that support resident comfort and engagement.

Observe behavioral or mood changes that may indicate unmet needs such as pain, infection, medication effects, or environmental stressors, and collaborate with the care team to address underlying causes.
Staff Support & Floor Leadership

Provide daytoday clinical support to Care Partners and Medication Technicians.

Partner with the Director of Memory Care in guiding staff through complex care situations.

Assume a lead clinical role on the floor.

Assist staff in responding to behavioral or medical situations requiring clinical guidance.

Promote teamwork and accountability in delivering highquality care.

Support interdisciplinary collaboration between clinical, caregiving, and activities teams.

Build strong, trusting relationships with residents and their families.

Serve as a knowledgeable and approachable resource for families regarding resident health and wellbeing.

Communicate professionally and compassionately about changes in resident condition.

Support family understanding of dementia progression and care approaches.

Help create an environment where families feel informed, reassured, and connected to the care team.

Active Florida Licensed Practical Nurse (LPN) license in good standing.
-
Previous experience in Memory Care, Alzheimer’s care, or dementia care required.

Experience in assisted living, longterm care, or senior living communities preferred.

Strong understanding of dementiarelated behaviors and dementiainformed care practices.

Certified Dementia Practitioner (CDP) certification preferred or willingness to obtain.

Strong clinical judgment and ability to recognize changes in resident condition.

Ability to support and guide caregiving staff in complex care situations.

Excellent communication and interpersonal skills.

Ability to build trust with residents, families, and team members.

Ability to assist residents with mobility and remain active throughout the shift.

Ability to perform essential job functions with or without reasonable accommodation.

Ability to pass a drug and alcohol screening.

Ability to pass a Level 2 background screening.
