Program Managers:
Works directly under supervision of the Program Director, receiving instructions and guidelines from the same. Has flexibility to plan and organize work assignments within guidelines of program structure.
Job Summary:
Under the supervision of Program Director, PM is responsible for oversite of all assigned program case load, development and implementation of all programs and services per Individuals Service Plan.
General Requirements:
Degree in related field and two years of experience or combination and experience in the field of developmental disabilities. Must be able to identify programmatic needs and generate appropriate solutions. Knowledge of Colorado IDD rules and regulations for Individuals with developmental and Intellectual Disabilities. Excellent writing skills, good organizational and communication skills, self-motivated and self-disciplined; Show proficiency in scheduled and required trainings, knowledge and proficiency in basic computer skill and applied software and programs, (e.g. excel, Therap, Relias, Dropbox etc.) ability to complete assigned tasks with minimal supervision, working knowledge of all program compliance and maintaining documentation associated with services provided. Must be willing and available to respond to crisis and emergency situations, which requires evening and weekend responsibilities. This position includes working with individuals with challenging behaviors and high medical needs. Clean, valid Colorado driver’s license and insurance. Must be able to prioritize job duties and work as member of team.
Description of Program Manager Duties:
1. Develop and implement Person Centered ISSPs, behavioral plans, nutrition, safety and care plans, protocols, per Individual’s Service plan. Complete/Update Assessments & Reviews Comprehensive Life Reviews/Functional Analysis. Program Director will review and approve all plans prior to implementation.
2. Ensure services are provided in accordance with service descriptions and compliance of rules with IDD/state, federal regulations, city, as well as company policy and procedures.
3. Attend all Service Plan and IDT meetings for individuals; ensuring completion of all required annual documentation as requested by CCB. Schedule and facilitate six-month program reviews.
4. Maintain up-to-date program files, database and documentation with all pertinent information, completing and updating Individual Data, Emergency Face Sheet, Safety Information and shared contacts as needed.
5. Review and approve incident reports, distribute to appropriate parties, identify trends and provide necessary follow-up and plan of action to prevent future incidents. Ensure Critical Incident Reports are completed and submitted within required timelines.
6. Complete Human Rights Committee (HRC) reports as assigned, submitting to Program Director for review prior to turning in to CCBs, and provide follow-up/response to HRC recommendations.
7. Ensuring all enrollment documents are completed and received.
8. Facilitate new program enrollments, residential and respite placements, coordinate all move plans, conduct appropriate monitoring and provide trainings, Conduct 30-day review.
9. Coordinate with other departments, day program, nursing and other professionals involved with the approved plan to assure individual in services’ needs and plan goals are met.
10. Conduct bi-monthly or quarterly program monitoring, including service site and home visits, medical monitoring, Home and Office files, completing programmatic and environmental inspections. ensuring that the homes are maintained in a clean, healthy and safe manner as per HUD requirements, individual nutritional needs, supplies, maintenance, and a face to face visit with the individual.
11. Recruit Host Home Providers, Independent Contractors and staff, to include, posting job openings, conducting interviews, checking references and submitting needed items for personnel file.
