Position Overview
Gracey Care is seeking a highly organized and detail-oriented Care Coordinator to join our team as a 1099 independent contractor operating within Arizona. The successful candidate must possess a medical background (such as CNA, MA, LPN, RN, or EMT) to ensure a strong command of medical terminology and the ability to provide clear medical education to our patients. Additionally, candidates must have demonstrable experience with Athena EMR and proficiency in PIN code navigation. This role requires superior communication skills, a thorough understanding of Medicare patient care, and the ability to manage multiple priorities efficiently.
Key Responsibilities
- Coordinate comprehensive care plans for Medicare patients, acting as a liaison among patients, family caregivers, healthcare providers, and insurance entities
- Accurately document patient information and track health outcomes utilizing Athena EMR
- Manage PIN code processes to verify insurance eligibility, benefits, and prior authorizations
- Provide expert guidance to patients regarding their care plans, benefits, and provider networks
- Collaborate with clinical teams to identify and address patient care needs proactively
- Ensure meticulous documentation and compliance with healthcare regulations
- Facilitate coordinated transitions of care to enhance overall patient satisfaction and health outcomes
Qualifications
- Proven experience with Athena EMR systems is essential
- Proven knowledge of medical terminology (candidate will be tested)
- Proficiency navigating PIN code systems related to healthcare insurance verification
- Experience coordinating care for Medicare populations preferred
- Exceptional verbal and written communication skills with a patient-focused approach
- Strong organizational skills and the ability to manage multiple tasks independently
- Ability to work autonomously as a 1099 contractor with effective time management
- Knowledge of Medicare guidelines and healthcare regulatory compliance is advantageous
