Responsibilities
- Conduct thorough patient assessments, including vital signs, physiological evaluations, and symptom management to develop personalized care plans.
- Manage complex cases by utilizing your critical care experience, ICU background, or emergency medicine expertise to address acute symptoms and stabilize patients as needed.
- Coordinate discharge planning and post-acute care services to ensure seamless transitions between hospital and home settings.
- Document all patient interactions accurately within electronic health records (EHR) systems while maintaining strict compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams—including physicians, social workers, and therapists—to deliver holistic patient service that meets medical and emotional needs.
- Educate patients and families on disease processes, medication management, and symptom control strategies to promote understanding and comfort.
- Participate in utilization management processes by evaluating patient needs against healthcare policies and ensuring appropriate resource allocation.
Qualifications
- Valid nursing license with current registration; experience in hospice or palliative care preferred but not required.
- Extensive hospital experience including ICU, Level I Trauma Center, Level II Trauma Center, or medical-surgical units.
- Proven expertise in critical care settings such as emergency room, PICU (Pediatric Intensive Care Unit), or OR (Operating Room).
- Strong knowledge of physiology, infectious disease management, health regulation policies, and medical terminology.
- Experience with electronic health records (EHR), discharge planning, case management, and outpatient or clinic-based nursing.
- Ability to perform comprehensive patient assessments including vital signs and diagnostic evaluations.
- Skilled in managing patients as a nurse case manager with a focus on managed care principles and utilization management.
- Excellent patient service skills with a compassionate approach tailored to geriatrics, behavioral health, pediatrics or post-acute populations.
