Physical Therapist (PT) (Contract)

Location: CA 95476
Start Date: ASAP
Duration: 13 Weeks
Schedule: 36 hours/week
Setting: Skilled Nursing Facility (SNF)
Employment Type: Travel Contract
Pay: $90+/hour

Position Summary & Responsibilities

Provide PT evaluations and individualized treatment plans for SNF patients
Deliver therapeutic interventions to improve mobility, strength, and function
Monitor progress and adjust care plans as needed
Document treatments and patient outcomes accurately
Collaborate with interdisciplinary care team
