Polaris Rehabilitation & Care Center is seeking compassionate, dependable, and resident-focused Certified Nursing Assistants (CNAs) to join our nursing team.
As a CNA, you will provide direct, hands-on care to residents while assisting with activities of daily living, mobility, personal care, and other essential needs. You will work closely with nurses and other members of the interdisciplinary team to help ensure each resident receives safe, respectful, and individualized care.
Our CNAs are an essential part of our clinical team. This position is ideal for someone who enjoys building relationships with residents, working as part of a team, and making a meaningful difference during every shift.
Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and personal hygiene
Obtain and accurately record vital signs, including temperature, pulse, respirations, and blood pressure
Observe residents for changes in physical condition, behavior, mood, mobility, appetite, skin condition, or overall well-being and promptly report concerns to the supervising nurse
Assist residents with transfers, ambulation, repositioning, and mobility equipment in accordance with individualized care plans and established safety procedures
Respond promptly and professionally to call lights and resident requests
Assist with meal service and provide feeding assistance when required
Provide routine skin care, incontinence care, oral care, and other personal care services within the CNA scope of practice
Accurately document care and services provided in accordance with facility procedures
Follow resident-specific care plans and communicate concerns or changes to licensed nursing staff
Maintain resident rooms and care areas in a clean, safe, and organized condition
Follow infection prevention, fall prevention, transfer, skin integrity, and resident safety procedures
Protect resident privacy, dignity, independence, preferences, and rights
Communicate respectfully with residents, families, visitors, nurses, and interdisciplinary team members
Follow all facility policies and applicable federal and Wyoming requirements
Current, active Certified Nursing Assistant (CNA) certification in the State of Wyoming
Certification must be in good standing with no disqualifying findings on the applicable Nurse Aide Registry
Previous experience in skilled nursing, rehabilitation, long-term care, assisted living, or another healthcare environment preferred but not required
Strong communication and interpersonal skills
Ability to follow individualized resident care plans and nursing direction
Dependable and able to work effectively as part of a clinical team
Strong observation skills and ability to recognize and report changes in resident condition
Ability to prioritize responsibilities in a fast-paced healthcare environment
Commitment to providing compassionate, respectful, and resident-centered care
Ability to perform the essential functions of the position with or without reasonable accommodation
