Job Summary
We are seeking a dedicated and skilled Home Health Physical Therapist Assistant to join our compassionate team. The ideal candidate will possess a strong foundation in physiotherapy practices, with experience in patient care across various settings such as outpatient clinics, inpatient facilities, and skilled nursing environments. As a vital part of our home healthcare team, you will assist licensed physical therapists in delivering personalized care plans that promote recovery, mobility, and independence for our patients.
Responsibilities
- Implement prescribed therapeutic interventions under the supervision of a licensed physical therapist, including trigger point therapy and other manual techniques.
- Assist patients with mobility exercises, functional training, and activities aimed at improving strength and flexibility.
- Monitor patient progress and document responses to treatment accurately in accordance with healthcare regulations.
- Support discharge planning efforts by preparing patients for transitions back to their homes or other care settings.
- Educate patients and their families on exercises, safety precautions, and injury prevention strategies tailored to individual needs.
- Maintain a safe, clean, and organized treatment environment that adheres to all health and safety standards.
- Collaborate with interdisciplinary teams to develop comprehensive care plans that address specific conditions such as orthopedics, geriatrics, pediatrics, or post-surgical rehabilitation.
Requirements
- Valid certification as a Physical Therapist Assistant (PTA) with current licensure or registration as required by state regulations.
- Proven experience working in home health settings; inpatient or skilled nursing facility experience is highly desirable.
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy.
- Familiarity with discharge planning processes and managing care across outpatient clinics or acute care environments.
- Experience working with diverse patient populations including geriatrics and pediatrics.
- Ability to develop and follow detailed care plans while demonstrating excellent patient care skills.
