About the Role
HouseCall MD is hiring a Nurse Practitioner to join our weekly rounds in the Lancaster area. You'll be seeing patients where they're most comfortable — at home or in a facility — and building the kind of relationships that actually make a difference in someone's health. This is per diem work with a flexible schedule, designed for an NP who wants clinical freedom without the chaos of a traditional clinic.
No rotating nights. No revolving-door appointments. Just focused, meaningful care.
What You'll Do
- Conduct weekly primary care rounds for patients in home and facility settings
- Develop and manage care plans tailored to each patient's situation
- Work through medical records and make sound clinical calls independently
- Stay in close contact with patients, families, and the broader care team
- Help patients genuinely understand their conditions and what comes next

Qualifications
Required:
- Active Nurse Practitioner license in California
- 2+ years of primary care experience
- Sound clinical judgment and strong communication skills
Preferred:
- Background in home health, geriatrics, or palliative care
- A natural ability to connect with patients and their families over time
Schedule
- Weekly rounds format, part-time
- Day shift with flexible scheduling
