Apex Health Solutions LLC is a licensed home care agency serving clients throughout Bergen County and surrounding areas in New Jersey. We are currently seeking dedicated Registered Nurses (RNs) to join our growing team and provide skilled nursing services to private-pay clients. The ideal candidate is compassionate, reliable, organized, and passionate about helping individuals maintain their independence and quality of life at home.
Responsibilities
- Conduct comprehensive nursing assessments and ongoing patient evaluations.
- Develop, implement, and update individualized care plans.
- Monitor and document patient health status and progress.
- Administer medications and treatments as ordered by physicians.
- Educate patients and family members regarding disease management, medications, and home safety.
- Coordinate care with physicians, caregivers, and other healthcare professionals.
- Maintain accurate and timely clinical documentation.
- Identify changes in patient condition and communicate findings to the Director of Nursing.
- Ensure compliance with agency policies, state regulations, and professional nursing standards.
- Participate in care conferences and quality improvement activities as needed.
Qualifications:
- Active New Jersey Registered Nurse (RN) License.
- Minimum 1 year of nursing experience preferred.
- Home care, hospice, geriatric, or community health experience preferred.
- Current CPR/BLS Certification.
- Strong assessment, communication, and documentation skills.
- Valid driver's license and reliable transportation.
- Ability to work independently and manage patient care effectively.
Pay: $45.00 - $55.00 per hour
