Job Summary
We are seeking dedicated Registered Nurses (RNs)and Licensed Practical Nurses (LPNs)to provide overnight skilled monitoring in the home setting. This role supports medically fragile children with developmental delays, complex medical needs, and families enrolled in GAPP (Georgia Pediatric Program).
Shifts typically include is 8.00pm-6.00am
Duties
- Provide comprehensive pediatric assessmentsin the home, including monitoring vital signs, evaluating developmental and functional status, reviewing medical histories, and identifying changes in condition that require intervention.
- Administer prescribed medications and treatments safely, including CPAP monitoring, inhalers/nebulizers, injections, G-tube/J-tube feedings, seizure rescue medications, and other routine or PRN interventions.
- Support chronic condition managementfor children diagnosed with epilepsy, sickle cell disease, asthma, sleep disorders, thalassemia, cerebral palsy, developmental delays, and other medical complexities.
- Deliver age-appropriate carethat promotes comfort, safety, and emotional well-being—engaging children in routines, therapeutic activities, and communication at their developmental level.
- Document all care thoroughlyusing EMR systems to ensure accurate communication, timely updates, and alignment with care plans, provider directives, and GAPP requirements.
- Perform skilled nursing procedures in the home, including wound care, G-tube care, suctioning, trach care (if applicable), oxygen support, catheterization, and other specialty interventions within scope.
- Collaborate closely with familiesto educate caregivers, reinforce care plans, review medication schedules, identify safety risks at home, and ensure they feel confident and supported.
- Coordinate with schools and providerswhen needed, supporting care transitions, attending IEP-related discussions (as permitted), and aligning with physicians, therapists, and case managers.
- Promote safety and prevent hospital readmissionsby monitoring early warning signs, teaching families preventive strategies, and escalating concerns promptly to clinical leadership.
