Delivers high-quality primary and preventive care for adults in an integrated community-mental-health setting, focusing on chronic-disease management, wellness promotion, and collaboration with behavioral-health teams.

Conduct full health assessments, diagnose & manage acute/chronic conditions.

Provide medication management, patient & family education, and minor procedures.

Order/interpret diagnostic testing.

Collaborate with physicians, therapists, case managers; mentor nursing staff.

Maintain thorough EHR documentation and participate in qualityimprovement initiatives.

MSN or DNP, national NP board certification (AANP/ANCC).

Active IL NP license; Controlled Substance/DEA & FullPractice Authority preferred.
- 12 yrs primarycare NP experience; behavioralhealth exposure valued.

Proficiency with EHR (athenaOne) and strong teamwork/communication skills.

Work Environment & Physical Requirements

Ambulatory clinic environment with some standing/walking; must meet standard clinical credentialing and vaccination requirements.
