Cornerstone Community Initiative (CCI) is seeking a compassionate, organized, and mission-driven Reentry Case Manager/Navigator to help justice-involved individuals successfully transition from incarceration back into the community.
This is more than a case management position. As Monroe County's coordinated reentry hub, CCI works alongside correctional facilities, probation, behavioral health providers, employers, housing partners, and community organizations to ensure individuals receive the support they need to rebuild their lives. You'll serve as the connector who helps participants navigate complex systems, coordinates services across multiple agencies, and ensures no one falls through the cracks.

Job Overview
The Reentry Coordinator provides participant-centered reentry planning, resource coordination, and system navigation for individuals before and after release from incarceration. Working closely with participants and community partners, you'll coordinate services, track referrals and outcomes, strengthen partnerships, and help ensure participants remain engaged with the supports they need for long-term success.
Key Responsibilities
- Conduct participant assessments and develop individualized reentry plans.
- Coordinate and track referrals across partner organizations while monitoring participant engagement and follow-through.
- Build collaborative relationships with correctional staff, probation, service providers, employers, and community organizations.
- Maintain accurate participant documentation, data, and outcome tracking.
- Advocate for participants while helping remove barriers to housing, employment, education, healthcare, recovery, and other community supports.
- Represent CCI at community meetings, coalition activities, and outreach events.
Qualifications & Skills

- Organized, compassionate, and highly collaborative.
- An excellent communicator who enjoys building relationships.
- Comfortable coordinating services across multiple organizations.
- Able to manage documentation while balancing participant engagement.
- Committed to professionalism, accountability, dignity, and respect.
- Experienced in human services, criminal justice, behavioral health, case management, recovery support, or a related field.
Bachelor's degree preferred; equivalent combinations of education, professional experience, volunteer experience, and lived experience will be considered.

Pay: $26.00 per hour
