Job Title: Licensed Social Worker / Discharge Planner
***Must hold a valid Social Worker license in Texas***

Duties:
· Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of this facility
· Assist in the development, administering, and coordinating of department policies and procedures
· Assist in discharge planning, including but not limited to acquiring durable medical equipment, referring to Hospice and Home Health agencies, arranging transportation into community, finalizing a discharge location (i.e., home, apartment, Assisted Living Facility)
· Conduct assessments related to cognition and mood
· Document in Medicaid affiliated online server
· Oversee and refer to ancillary departments such as audiology, dentistry, optometry, podiatry, psychology/psychiatry
· Organize and conduct quarterly Care Plan meetings with residents and their families
· Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Director or Administrator
· Review department policies and procedures, at least annually, and participate in making recommended changes
· Participate in community planning related to the interests of the facility and the services and needs of the resident and family
Qualifications:
· Social Work license is required
· Long Term Care / Skilled Nursing Facility experience preferred but open to new graduates
· Must have, as a minimum, a bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology.
