Job Overview
We are seeking a dedicated Registered Nurse (RN) to provide high-quality home care services within healthcare facilities. This role offers an exciting opportunity to deliver personalized nursing care in various clinical settings, ensuring patient safety, comfort, and optimal health outcomes. As an integral part of our team, you will coordinate care plans, perform assessments, and support patients through their recovery and ongoing health management. This role will also include getting to know residents and being the face of Home Care of Michigan at the facility. It will also include running weekly blood pressure clinics for residents. Candidate must be outgoing, friendly and approachable.
Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews.
- Administer medications, injections, IV infusions, and perform catheter care following aseptic techniques.
- Manage complex clinical procedures such as wound suturing, ostomy care, and ventilator management.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with patient needs.
- Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner while maintaining HIPAA compliance.
- Provide education on disease management, medication administration, nutrition, and self-care strategies to patients and their families.
- Monitor for signs of acute pain or infectious diseases and respond promptly with appropriate interventions.
Skills
- Proven experience in hospital medicine, ICU, emergency medicine, or post-acute care environments.
- Proficiency with EMR/EHR systems such as Epic or Cerner for accurate documentation.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology.
- Skilled in patient assessment techniques including vital signs measurement and diagnostic evaluation.
- Experience working with individuals with disabilities or in memory care settings is highly desirable.
- Ability to perform advanced clinical procedures such as tube feeding management, laboratory specimen collection/processing, and sterile processing.
- Critical care experience including Level I or Level II trauma center background is preferred. Pay: $65.00 - $110.00 per hour
