MAJOR DUTIES AND RESPONSIBILITIES:
Clinical Services and Care Coordination
· Patient screening, intakes, assessment for appropriate ASAM level of care, treatment planning, orientation to treatment, group and individual counseling, case management, facilitate referrals, discharge planning and consultation
· Facilitate appropriate referrals based on ASAM level of care (e.g. IOP, PHP, sober level, residential, housing, etc.)
· Innovative program development and implementation
· Incorporate neuro-science informed psychoeducation (e.g. reward pathways, habit loops, craving cycles) to empower client insight and behavior change
· Support both abstinence-based and risk reduction goals based on patient preference and clinical appropriateness
Group Facilitation
· Design and facilitate dynamic, process-oriented and psychoeducational groups for individuals with Alcohol Use Disorder (AUD) and other co-occurring disorders
· Integrate cutting edge topics into groups (e.g. nervous system regulation and stress response, values in recovery, mindfulness, shame resilience and relapse prevention)
· Foster a safe, inclusive and confidential group environment that encourages connection and accountability
Medication Management & Care Coordination
· Educate patients on medication options for AUD and co-occurring disorders, and support patient centered decision-making
· Provide referrals to both internal and external providers for medication evaluation, physician assisted treatment and ongoing medication management
Documentation and Compliance
· Demonstrate understanding of and adherence to appropriate ethical behavior as stipulated in the CODAC Policy and Procedures Manual
· Demonstrate understanding of and adherence to confidentiality regulations as stipulated in the CODAC Policy and Procedures Manual, Federal Regulation 42CFR part 2 and other pertinent Federal and State laws and regulations
· Maintain individual and group caseloads in accordance with CODAC procedures
· Monitor urine and breathalyzer tests when necessary
· HIV pre/post-test counseling
· Obtain and/or maintain appropriate credentialing and licensure
· Provide case presentations at clinical meetings
· Obtain and maintain Motivational Interviewing proficiency
· Participate in Performance Improvement activities
· Perform other duties as required
Crisis Intervention
· Assess and respond to relapse risk, withdrawal symptoms, and mental health crises.
· Conduct risk assessment and implement safety planning as needed.
· Facilitate appropriate referrals for detoxification and emergency care.
QUALIFICATIONS:
Master’s Degree LMHC, LCDP and/or LICSW and 3-5 years’ experience in the field. Bilingual required.
