Sinai Urban Health Institute (SUHI) is hiring a culturally competent, bilingual Community Health Worker (CHW) II with previous case management or care coordination experience. In partnership with Sinai Community Institute and Sinai Behavioral Health, CHW services focus on trauma-informed, culturally competent care, providing case management, aiming to reduce violence and improve client access to social and behavioral health resources.
Under direction of the department supervisor, the CHW is responsible for supporting individuals, assessing social and clinical needs, assisting individuals with navigating the health and social care systems, providing case management services, and documenting detailed case notes for each interaction. They act as a bridge between the patient, community, and medical system by building trusting relationships with community members served by the hospital or program(s). The CHW also helps to identify barriers to accessing quality care, works with individuals to overcome these barriers, and provides referrals. They will be required to collaborate with external partners, attend meetings and may work in a fast-paced environment such as the emergency department. This position is fully in-person for the CHW to work closely with the care team and clients.

Position Requirements:
- Bilingual in English and Spanish
- Must have high school diploma or GED
- 2 years of experience in case management and/or care coordination with violence prevention services, preferred
- Knowledge of Chicago’s West Side and/or South Side communities (Resident of West Side or South Side communities preferred)
- Ability to work in a team environment
- Passion for and dedication to working with community members and providers
- Punctual, reliable and willing to learn
- Demonstrated commitment to racial and health equity
- Must have a valid driver’s license and vehicle
- Proficiency in Microsoft Word, Excel, Access and PowerPoint preferred
