Job Description
The Social Care Network Specialist will provide focused outreach and services to Medicaid eligible neighbors across Syracuse through the Social Care Network. The Specialist will complete screenings and assessments with neighbors and provide Enhanced Care Management services that support neighbors in overcoming barriers to health, wellbeing, and opportunity.
Essential Job Duties and Responsibilities
- Complete Unite Us trainings and qualifications within 2 weeks of hire
- Become expert in Healthy Alliance Social Care Network systems and maintain expertise through program changes and updates
- Attend all Healthy Alliance trainings, meetings, and updates
- Assist SNCC-NEHDA staff with screenings and assessments as well as enhance care management navigation
- Work collaboratively with SNCC-NEHDA staff, coordinate with other SNCC-NEHDA services
- Perform at least 2 outreach events/contacts each week
- Complete 50 screenings and assessments per month
- Complete Enhanced Care Management services
- Duties will require meetings and attendance outside of 9-5 hours; the Specialist will be able to flex hours so as not to exceed 40 hours per week
- Other duties as assigned by Executive Director, Deputy Director, and Supervisor

Minimum Qualifications
Requirements listed represent necessary abilities and skills. Modifications may be made to enable all individuals to perform the essential functions.
- Experience in health and human services delivery preferred
- Syracuse City resident with strong community connections preferred
- Ability to self-direct, manage and prioritize tasks, and manage time required
- Professional, welcoming, compassionate demeanor required
- Must possess reliable transportation for use within Onondaga County
Certificates and Licenses
- Valid and clean NYS driver’s license
- Ability to pass criminal and DMV checks upon hire
Equipment, Machines, and Software
- Regular use, care, and security of computer, copier, and cell phone
- Software includes MS Office, Salesforce, online databases, and others
Working Conditions and Physical Demands
- This position requires the ability to travel throughout the service area
- There may be occassional exposure to outside weather
- The position requires the ability to work at a computer for extended periods and to attend meetings
