Mindify Wellness & Care is seeking an outgoing, professional, and relationship-driven Community Outreach Representative to expand our community presence throughout all of New Jersey. This role serves as a key ambassador for Mindify, introducing our services to adult day care centers, physician offices, and community organizations, identifying referral opportunities, scheduling partnership meetings with leadership, and fostering long-term relationships through ongoing in-person engagement.

Travel throughout New Jersey visiting adult day care centers, physician offices, senior centers, and community organizations.

Introduce Mindify's behavioral health services and educate potential referral partners on the value of our programs.

Build meaningful relationships with administrators, Directors of Nursing (DONs), physicians, office managers, social workers, and case managers.

Identify qualified partnership opportunities and schedule meetings with Mindify leadership.

Represent Mindify at networking events, community gatherings, and industry functions.

Conduct regular in-person check-ins with existing referral partners.

Maintain strong communication and cultivate long-term professional relationships.

Coordinate follow-up activities with leadership and clinical staff as needed to support partner success.

Deliver presentations highlighting Mindify's services and impact.

Educate partners on referral processes, behavioral health resources, and available support services.

Accurately document outreach activities, meetings, referrals, and follow-up efforts within the CRM system.

Submit weekly outreach reports and provide market insights and feedback.

Bachelor's in Public Health, or equivalent

Excellent communication, interpersonal, and relationship-building skills.

Valid driver's license and reliable transportation.

Willingness to travel throughout New Jersey

Experience in healthcare outreach, marketing, customer service, community engagement, or business development preferred.

Salary: $55,000–$60,000 annually, based on experience.

Eligible for a semi-annual performance bonus of up to $5,000 every six months (up to $10,000 annually).
