Responsibilities
- Provide comprehensive nursing assessments and develop individualized care plans for patients receiving home health or hospice services
- Administer medications, treatments, and interventions following physician orders while monitoring patient responses
- Educate patients and families on disease management, medication usage, symptom control, and post-acute care procedures
- Coordinate with interdisciplinary teams to ensure seamless discharge planning, case management, and utilization review
- Conduct regular visits to evaluate patient progress, vital signs, and physiological status; adjust care plans accordingly
- Maintain accurate medical records in compliance with HIPAA regulations and document all patient interactions thoroughly
- Collaborate with physicians, social workers, therapists, and other healthcare providers to optimize patient outcomes in outpatient or inpatient settings
Requirements
- Valid RN (Registered Nurse) license in good standing with relevant state licensing boards
- Strong background in hospital medicine, medical-surgical units, or post-acute care settings including outpatient clinics or home health agencies
- Knowledge of physiology, medical terminology, infectious disease protocols, and discharge planning processes
- Experience with patient assessment techniques, vital signs monitoring, diagnostic evaluation, and case management strategies
- Familiarity with HIPAA compliance standards and medical records documentation practices
- Ability to work independently while managing multiple patient cases efficiently in a fast-paced environment and communicating with the each patients' healthcare team
