Position Summary:
Laurelwood Nursing and Rehab Center is seeking a compassionate and dedicated Licensed Social Worker to join our interdisciplinary team. In this role, you will provide essential support to residents and their families by addressing their psychosocial needs and ensuring the highest quality of care and quality of life.
Responsibilities:
Conduct comprehensive psychosocial assessments and develop personalized care plans
Offer counseling and emotional support to residents and their families
Coordinate discharge planning and transitions of care to ensure seamless transitions
Serve as a liaison between residents, families, and care teams to ensure effective communication
Assist with admissions and resident adjustment to promote a smooth transition
Ensure compliance with state and federal regulations to maintain the highest standards of care
Qualifications:
Licensed Social Worker (LSW/LCSW) in Maryland
Prior experience in a skilled nursing facility is preferred
Strong communication and interpersonal skills are essential
