Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient observations including vital signs, mental status, and overall health condition; document findings accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Support medication administration following medical documentation protocols and ensuring compliance with HIPAA regulations
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care strategies
- Assist with medical procedures such as catheter care, hospice support, and working with patients with developmental disabilities or disabilities
- Prepare nutritious meals and assist with meal planning while maintaining proper food safety standards; perform light housekeeping tasks including cleaning and laundry to promote a hygienic environment
- Maintain detailed medical records and documentation of patient care activities to ensure continuity of care and compliance with healthcare regulations
Qualifications
- Prior experience in home health care, assisted living facilities, nursing homes, or long-term care environments is highly preferred
- Knowledge of medical terminology, vital signs measurement, and medical documentation practices
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or dementia-specific certifications are a plus
- Experience working with individuals with disabilities or developmental challenges is advantageous
- Familiarity with EMR/EHR systems for accurate record keeping and patient data management
- Ability to work independently while following established care plans and protocols
- Compassionate attitude combined with excellent communication skills to build trusting relationships with clients and their families

Pay: $15.00 - $20.00 per hour
