Southern California Crossroads is a non-profit organization that serves low-income communities with violence prevention, victim services, youth development, and other social support services.
We are looking for a motivated Case Manager with experience to work out of our Lynwood location. We are looking for a motivated person with excellent case management, documentation skills, and experience working with clients that are affected by gang violence, human trafficking, domestic violence, mental health, and substance abuse. Locations of service are in hospital, their home, our offices, and in the community.
Overview of position: HBVI Case Manager will be responsible for engaging potential clients who are victims of crime in the hospital and enrolling them into the program, providing bed-side counseling and victims of crime resources while the patient is admitted and continue with case management post discharge. The case manager work with clients to develop an individualized service plan. This will be done through case management, community accompaniment, resource navigation, and mentorship. Client cases will vary as some will require intensive work while others will be more standard cases.
Duties include:
- Case Management
- Bedside counseling
- Engage hospital patients who are victims of violence and introduce them to services
- Assessing the needs of the client
- Creating an individualized service plan
- Conduct a needs assessment with clients to evaluate existing risk-factors
- Use the needs assessment to develop an individualized service plan for clients to heal and recover from trauma with a focus on addressing issues such as unemployment, job training, housing and food insecurity, substance abuse, mental health needs and symptoms of post-traumatic stress, access to public benefits, victims of crime application, educational attainment, and relocation
- Conduct case management and community accompaniment to help clients navigate their recovery
- Provide crisis intervention
- Usher your clients to a place of healing from trauma
- Provide mentorship to clients, modeling and supporting positive growth
- Visit patients and interact with families
- Advocate for the client when needed
- Regularly input client data into the electronic program database
- Getting your clients involved in positive activities
- Providing consistent support to clients and their families
- Making referrals to other agencies when appropriate
- Maintaining accurate records and preparing reports
- Participating in training, supervision, and meetings
- Participate in trainings for personal and professional development, including self-care and wellness
- Represents the organization at various agency and community meetings.
- Work with a team of medical, and social service staff
- Work closely with hospital and community partners to build a network of social service resources
- Ensure the academic and employment success of your clients
- Ensure the positive development of your client
- Participate in training and self-care activities when provided by Crossroads
- Maintain healthy self-care for your personal well being
- Maintain positive and consistent ‘business casual’ dress code
- Conduct the daily tasks given to you by your supervisor
- Assists with general cleanliness of the office and workstation
- Work a flexible schedule, which includes weekend shifts
