The Dental Outreach Coordinator is responsible for promoting access to oral health services by building relationships with community partners, coordinating outreach events, and supporting patient engagement for dental services. This role focuses on increasing utilization of dental services among underserved populations, improving oral health literacy, and supporting FQHC mission-driven goals. A significant component of this role includes proactive telephone outreach to existing medical patients to educate them on available dental services, address barriers to care, and facilitate connection to dental appointments.

Conduct outbound and inbound telephone outreach to existing medical patients to connect them with dental services.

Educate patients on the importance of oral health and the integration of dental care within the FQHC.

Assist patients with scheduling dental appointments and navigating eligibility, insurance, and sliding fee scale processes.

Identify and address common barriers to dental care (transportation, language, fear, availability) and coordinate with care teams as needed.

Document outreach activities, patient interactions, and outcomes in the electronic health record or tracking systems.

Collaborate with medical care teams, referral coordinators, and dental staff to support referrals between medical and dental services.

Support integration of dental outreach with medical and behavioral health.

Work closely with dental providers and assistants to support outreach goals.

Develop and maintain relationships with community organizations, schools, Head Start programs, shelters, senior centers, and other local partners.

Coordinate and participate in dental outreach events such as health fairs, school-based screenings, and community education sessions.

Identify barriers to dental care access and communicate community needs to dental leadership.

Collaborate with front desk, eligibility, and care teams to facilitate patient follow-up after outreach events.

ENVIRONMENT
The environment for this position is a medical clinic that is clean and comfortable. It may include some minor annoyances such as noise, odors, drafts, etc. The incumbent is in a non-confined office-type setting in which he or she is free to move about at will.

EDUCATION, TRAINING AND EXPERIENCE

To perform effectively in this position, the incumbent must have:

High school diploma or GED equivalent.

Prefer experience in an FQHC, dental setting, or school-based/mobile health program.

Familiarity with UDS measures, health education, or community health worker models.

Experience in telephone-based patient outreach, scheduling, or care coordination in a healthcare setting.

Effective oral, listening and writing communications skills in both English and Spanish.
-
Ability to work effectively with people of diverse cultures, ages and economic backgrounds.
Typically, these skills are the result of a combination of education and experience equivalent to a high-school degree or GED equivalent and several years of experience working in a community service organization preferably direct client services in a health-related organization.
The incumbent must be able to perform this job safely, without endangering the health or safety of him/herself or others.
