Location: Niles Primary Care
Schedule: Full-Time, Monday–Friday, 8:00 AM – 5:00 PM
About the Role
We are seeking a compassionate, bilingual Case Manager to join our Enhanced Care Management (ECM) program at Niles Primary Care. This is a rewarding opportunity to make a real difference in the lives of our patients by coordinating care, supporting health goals, and connecting them with the resources they need.
Responsibilities
- Manage a panel of ECM patients, ensuring timely follow-up and care coordination
- Collaborate with providers, specialists, and community resources
- Conduct patient outreach and home visits as needed
- Assist patients in overcoming barriers to care
- Maintain accurate and timely documentation in the EMR
Qualifications
- Bilingual (English/Spanish) – required
- Compassionate, patient-focused, and highly organized
- Previous healthcare, social work, or case management experience preferred
- Strong communication and problem-solving skills
