Home Health Registered Nurse (RN) Case Manager
Service Locations: Waukesha County and Lake Country
Employment Type: Full-Time (Exempt)
Pay Rage: $80,000-$85,000 per year
Position Summary
Here at BrightStar Care of Greater Waukesha and Lake Country, we are seeking a compassionate and skilled Home Health Registered Nurse (RN) Case Manager to join our home health team. In this role, you will provide highquality skilled nursing care, coordinate services, and support patients in achieving their highest level of independence at home. Key Responsibilities

Perform comprehensive initial and ongoing patient assessments in the home setting.

Develop individualized care plans based on patient needs, physician orders, and regulatory requirements.

Deliver skilled nursing interventions including wound care, medication management, IV therapy, catheter care, and diseasespecific education.

Monitor patient progress, identify changes in condition, and communicate updates promptly to physicians and interdisciplinary team members.

Coordinate services with nurses and caregivers to ensure continuity of care.

Complete timely, accurate documentation including assessments and visit notes.

Educate patients and caregivers on disease processes, medications, safety, and selfmanagement strategies.

Participate in case conferences, quality initiatives, and oncall rotation as assigned.
Qualifications
-
Current and unrestricted Registered Nurse (RN) license in the state of practice.

Five (5) years of clinical experience (Preferred)

Two (2) years of experience in a home health agency (Preferred)

Strong assessment, criticalthinking, and communication skills.

Ability to work independently, manage a fieldbased schedule, and prioritize multiple tasks.

Knowledge of Medicare home health regulations and documentation standards preferred.

Valid driver's license and reliable transportation.
