Job Title: Community Hospice RN Case Manager
Location: Grand Junction, CO
Contract Length: 12 Weeks
Schedule: Monday-Friday, 8am-5pm
Job Overview
We are seeking a compassionate and experienced Hospice RN Case Manager to provide exceptional end-of-life care for patients in their homes, nursing homes, assisted living facilities, and other community settings. This role is ideal for an RN who is passionate about delivering patient-centered care while supporting families through every stage of the hospice journey.
Key Responsibilities
- Manage an assigned caseload of hospice patients
- Perform comprehensive patient assessments and develop individualized care plans
- Provide symptom management and coordinate end-of-life care
- Educate patients and caregivers on treatment plans and home care
- Lead and collaborate with the interdisciplinary care team
- Maintain accurate clinical documentation and ensure quality patient care
Qualifications
- Active Registered Nurse (RN) license
- Hospice, Home Health, or Palliative Care experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and compassionate patient care skills
- Ability to work independently and manage a caseload effectively
Pay: $2,000.00 - $2,300.00 per week
