Position Overview
Comfort Home Health Care Services, LLC is a Minnesota DHS-licensed Home and Community-Based Services (HCBS) provider seeking a Minnesota-licensed Registered Nurse (RN) to serve in a part-time oversight, coordination, and compliance role.
This position fulfills requirements for both:
- 245D Designated Manager/Coordinator, and
- Community First Services and Supports (CFSS) Qualified Professional (QP)
The RN will support client intakes, progress reviews, 3-month assessments, annual care plan renewals, staff training, and person-centered service planning, ensuring full compliance with Minnesota DHS statutes, rules, and best practices.
This is a non-clinical, role-based position focused on oversight and regulatory compliance rather than routine bedside care.
Primary Responsibilities
245D Designated Manager/Coordinator Responsibilities
- Serve as the Designated Coordinator in accordance with Minn. Stat. §245D.081
- Coordinate and oversee service delivery to ensure services are delivered according to person-centered service plans
- Monitor service outcomes and ensure compliance with 245D licensing requirements, policies, and procedures
- Participate in quality assurance, service monitoring, and internal compliance activities
- Be available for DHS, lead agency, or internal consultation related to service coordination and compliance
CFSS Qualified Professional (QP) Responsibilities
- Provide professional oversight and consultation as a CFSS Qualified Professional
- Participate in and support client intakes and service planning
- Develop, review, and monitor care plans and progress documentation
- Review service delivery and documentation for compliance with CFSS program requirements
- Support service quality, health and safety oversight, and person-centered outcomes
- Maintain appropriate professional availability and communication
Clinical Oversight & Program Support
- Conduct or participate in new client intakes
- Complete and review 3-month assessments, progress reviews, and annual care plan renewals
- Support and review person-centered service plans in compliance with DHS requirements
- Ensure person-centered planning principles are followed and properly documented
- Provide or support employee training as required under DHS and 245D standards
- Review documentation to ensure regulatory, licensing, and audit readiness
Scope of Role
- No routine direct client care or shift coverage
- Role-based, not shift-based
- Hours fluctuate based on agency needs
- Some weeks may require minimal or no hours
- Primarily administrative, oversight, coordination, and compliance-focused
Work Schedule
- Average workload up to 3–6 hours per week
- Flexible scheduling
- Combination of remote work and on-site or virtual meetings as needed
Required Qualifications
- Active, unrestricted Minnesota Registered Nurse (RN) license
- Meets Minnesota DHS requirements to serve as a 245D Designated Coordinator, including:
- Licensed health professional authorized to coordinate and oversee service delivery under Minn. Stat. §245D.081
- Ability to coordinate services, monitor service outcomes, and ensure services are delivered in accordance with person-centered service plans
- Ability to support compliance with 245D policies, procedures, staff training, and quality assurance requirements
- Meets Minnesota DHS requirements to serve as a CFSS Qualified Professional (QP), including:
- Licensed Registered Nurse eligible to provide professional oversight and consultation
- Ability to participate in service planning, review service delivery, and monitor service quality under CFSS
- Ability to review documentation and support compliance with CFSS program requirements
- Knowledge of or ability to learn Minnesota DHS Home and Community-Based Services (HCBS) regulations
- Experience with client intakes, care planning, assessments, progress reviews, and staff training preferred
- Strong documentation, communication, and organizational skills
- Ability to work independently in a part-time, oversight-focused, compliance-driven role
Preferred Qualifications
- Experience working with culturally diverse populations
- Familiarity with DHS waiver services and person-centered planning

Compensation & Employment Details
- $1,500 per month salaried compensation
- W-2 employee status
