To oversee and lead the Patient Health Care Coordination Center (PHCC). To accurately and efficiently manage the triage of telephone calls received from patients, caregivers, family and other members of the healthcare team. To manage the prioritization of patient health concerns utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. To ensure, under the direction of the
patient's physician, that the education and advising of patients as well as the scheduling of appointments are made in a clinically safe and effective manner. To act as a clinical resource to staff for issue resolution. To ensure patient satisfaction goals are met.
Key Duties & Responsibilities:

Provides leadership to ensure effective and efficient daily operations of the Patient Health Care Coordination Center (PHCC). Assists management in development, implementation and evaluation of ongoing services that ensure quality patient care is provided consistent
with the UAB Medicine mission. Collaborates with other UAB Health System staff and marketing department to maintain an accurate referral database. -
Manages human resources functions for clinical and non-clinical staff including, but not limited to, staff selection, training, scheduling, evaluating and disciplining.

Monitors customer satisfaction and addresses concerns in a timely manner. Documents issues appropriately. Generates activity and accountability reports. Collaborates with management to develop action plans as needed. Leads ongoing performance improvement
initiatives. -
Oversees the management of physician's patient schedules.

Ensures compliance with external regulatory and accreditation requirements. Ensures compliance with all internal policies and procedures. Assists with the development and revision of departmental policies, protocols and procedures as required.

Performs other duties as assigned.
Bachelor's degree in Nursing and five (5) years of related experience, two (2) of which must be supervisory, required. Work experience may NOT substitute for education requirement. Currently licensed or licensed by endorsement as a Registered Nurse (RN) by the Alabama Board of Nursing required. Certified in Basic Life Support (BLS) from the American Heart Association (AHA) required.
