The Home Health Registered Nurse (RN) is responsible for providing skilled nursing care to patients in their private residences. This includes assessing patient needs, creating and implementing personalized care plans, and educating patients and their families. The RN will coordinate with physicians and other members of the healthcare team to ensure the highest quality of care for adult and geriatric patients with diverse needs, including those recovering from surgery, managing chronic illness, or requiring palliative care. Essential duties and responsibilities
- Patient assessment: Conduct comprehensive initial and ongoing assessments of patients' physical, psychosocial, and environmental needs, monitoring and documenting vital signs, symptoms, and overall health status.
- Care planning: Develop, implement, and update individualized, goal-oriented care plans in collaboration with the patient, family, and physician.
- Medication and treatment administration: Administer and monitor the effectiveness of medications, injections, IV therapy, and other treatments as prescribed by the physician.
- Wound and catheter care: Provide skilled wound care, including dressing changes and assessment of healing progress. Manage ostomy and catheter care as needed.
- Patient and family education: Instruct patients and caregivers on medication management, disease processes, wound care, pain management, and other home care procedures.
- Coordination of care: Act as a case manager by coordinating with the patient's physician, physical therapists, occupational therapists, social workers, and home health aides to ensure seamless, holistic care.
- Documentation: Maintain accurate, timely, and complete clinical records using electronic health records (EHR) in compliance with agency policies and regulatory standards.
- Communication: Communicate effectively with patients, family members, and the healthcare team to report changes in a patient’s condition and ensure continuity of care.
- Advocacy and support: Serve as a patient advocate, providing emotional support and guidance to patients and families during challenging times.
- Emergency response: Respond to and manage emergency situations that may arise during a home visit.
Qualifications
- Licensure: Current, active Registered Nurse (RN) license in the state of practice.
- Education: Associate Degree in Nursing (ADN) is required; Bachelor of Science in Nursing (BSN) is preferred.
- Experience: A minimum of 1–2 years of recent clinical experience, with home health experience strongly preferred. Other relevant experience may include medical-surgical, intensive care, or geriatric nursing.
- Certifications: Current CPR certification is required.
- Transportation: Reliable transportation, a valid driver's license, and proof of current auto insurance are necessary for travel to patient homes.

Pay: $65.00 per hour
