Speech-Language Pathologist (SLP/ST) — Home Health

Be More Home Health Services is a Medicare-certified, ACHC-accredited, women-owned home health agency built around a patient-education-centered model of care - helping patients recover safely at home while avoiding unnecessary rehospitalization. We're looking for a skilled, compassionate Speech-Language Pathologist to join our interdisciplinary team and help adult and geriatric patients regain communication, cognitive, and swallowing function in the comfort of their own homes.
Key Responsibilities
- Evaluate patients' speech, language, cognitive-communication, and swallowing abilities to determine level of function and treatment needs.
- Develop and implement individualized therapy plans as ordered by the attending physician, aligned with patient goals and medical history.
- Provide skilled treatment addressing speech, language, voice, cognitive, and dysphagia disorders.
- Instruct and train family members and caregivers in techniques to support the patient's communication and swallowing safety.
- Evaluate the home environment and make appropriate safety and functional recommendations.
- Observe, document, and report patient response to treatment; submit progress reports to the physician and agency at required intervals and at discharge.
- Collaborate closely with physicians, nurses, PT/OT, and case managers as part of the interdisciplinary care team.
- Participate in care conferences, in-service education, and staff development activities.
- Utilize community resources and appropriate referrals to support the patient's overall plan of care.
- Accurately document all evaluations, treatment, and progress notes in the EMR (WellSky) in accordance with agency, Medicare, and regulatory guidelines.
- Participate in Quality Assessment and Performance Improvement (QAPI) activities as assigned.
Qualifications
- Master's degree in Speech-Language Pathology (or equivalent).
- Current California license, registration, or certification as required to practice.
- Certificate of Clinical Competence (CCC-SLP) from ASHA, or eligible.
- At least 1 year of supervised work experience with adult patients; home health experience strongly preferred.
- Working knowledge of Medicare home health regulations and interdisciplinary care planning preferred.
- Strong assessment, documentation, and patient education skills.
- Comfortable working independently while staying closely connected to a small, collaborative clinical team.
- Proficiency with EMR and clinical documentation systems.
- Current driver's license, reliable transportation, and auto insurance for travel throughout San Diego County.
- Bilingual (Russian/English) a plus, not required.

Pay: $95.00 - $110.00 per hour
