Why we need you:
The Central Coordinated Access Network (CAN) Manager is responsible for the day-to-day management, coordination, and continuous improvement of the Coordinated Access Network (CAN) serving the Central Connecticut region. The position ensures that households experiencing homelessness are connected to appropriate housing interventions through a coordinated, equitable, data-driven, and housing-focused system. The CAN Manager works closely with providers, municipalities, state agencies, housing partners, outreach teams, shelters, and people with lived experience to improve system performance and housing outcomes. Coordinated Access serves as Connecticut's standardized entry point for homelessness services and housing resources.
What you will do:
System Coordination & Leadership
- Lead implementation and oversight of Coordinated Access operations across the Central CAN.
- Facilitate case conferencing, prioritization, system coordination, and other cross-system meetings.
- Ensure consistent application of Coordinated Access policies, procedures, and prioritization standards.
- Develop system improvements that increase efficiency, equity, and housing outcomes.
- Foster collaboration among providers, municipalities, state partners, and other stakeholders.
- Serve as the primary point of contact for CAN operations and lead Cold Weather coordination and emergency response.
Housing-Focused System Management
- Monitor housing placements, housing inventory utilization, and referral activity to maximize housing opportunities.
- Coordinate with housing authorities, landlords, supportive housing providers, and rapid rehousing programs.
- Manage Housing Authority preferences, voucher programs, set asides, and other housing resources.
- Review housing applications for eligibility and coordinate follow-up with providers.
- Identify barriers to housing and advance Housing First and person-centered practices.
Data, Performance & Quality Improvement
- Monitor CAN performance metrics and analyze HMIS, By-Name List, and Smartsheet data to improve system performance.
- Maintain accurate data, referral tracking, and reporting across required systems.
- Develop reports, dashboards, and presentations for funders, state agencies, municipalities, and community stakeholders.
- Ensure compliance with data quality standards, Coordinated Access requirements, and contract performance expectations.
- Lead continuous quality improvement initiatives and performance reviews.
Stakeholder Engagement
- Convene and facilitate provider meetings, workgroups, and community planning sessions.
- Build and maintain collaborative relationships with state agencies, municipalities, housing authorities, healthcare partners, and nonprofit providers.
- Coordinate training and technical assistance for CAN partners.
- Engage people with lived experience in system planning and evaluation.
- Represent Journey Home on local, regional, and statewide committees.
Strategic Planning & Innovation
- Support implementation of Journey Home's strategic plan and homelessness response initiatives.
- Lead special projects related to diversion, housing navigation, severe weather coordination, encampment response, and system redesign.
- Research best practices and identify opportunities to strengthen housing resources and system effectiveness.
Contract & Grant Management
- Monitor compliance with state and federal requirements and contract performance expectations.
- Assist with grant applications, contract deliverables, program monitoring, and required reports.
HUB Management
- Oversee daily HUB operations, including the Central HUB phone line, 211 coordination, appointment scheduling, and referral management.
- Facilitate shelter check-in meetings, case conferences, diversion manager meetings, and other system coordination activities.
- Support onboarding and training of diversion staff and maintain regular coordination with 211 and community partners.
Additional Supportive Roles
- Represent the Central and Greater Hartford CANs on committees and report on regional progress and challenges.
- Provide cross-coverage for CAN staff roles and assist with grants, advocacy, communications, fundraising, and the annual Point-in-Time Count.
- Perform other duties as assigned.
What you will bring:
- Bachelor’s degree with at least one year of experience in a related field preferred
- Bi-lingual in Spanish and English preferred
- Knowledge of federal and state funded housing programs preferred
- Intermediate or more advanced knowledge of Microsoft Office suite, including strong skills with Microsoft Excel, familiarity with Access preferred
- Excellent organization skills and attention to detail required
- Experience using Google Drive preferred
- Strong leadership and facilitation skills
- Knowledge of CT HMIS preferred
- Strong communication skills, including written correspondence and phone personality
- Ability to work independently, adjusting to changing priorities
- Ability to work with groups and balance multiple points of view

Compensation & Benefits:
- This is a full-time, exempt position
- Annual salary of $68,000-$75,000 per year; salary based on experience and qualifications, not salary history
