Overview
We are seeking a Licensed Clinical Social Worker to join our healthcare team. The ideal candidate will provide compassionate care and support to patients in need of mental health services. Job Description: You will deliver community stabilization services in a team. Services are provided in the individual’s natural environment and you will provide referrals and linkage to other community-based services at the appropriate level of care. Interventions may include, brief therapeutic and skill building interventions, engagement of natural supports, interventions to integrate natural supports in the de-escalation and stabilization of the crisis, and coordination of follow-up services. Typically each individual in the program can receive up to 28 hours of services per week.
You will need to be available during traditional business hours, as linkage with community partners is essential during service provision.
You will be responsible for completing the Individualized Service Plan, daily progress notes, discharge summaries and other clinical documentation as needed. You will also be required to attend mandatory trainings and supervisions.
You must be a team player, as you will be providing this service with a team member.
Job Requirements: You must be a LMHP or in residency . Additionally, our ideal candidate would exemplify professionalism, excellent communication and leadership skills, self-motivation, flexibility, integrity, strong clinical skills, strong computer skills and previous experience with an electronic documentation program and knowledge of DMAS, DBHDS and various MCO requirements.

Duties
- Conduct patient assessments to determine mental health needs
- Provide individual and group therapy sessions
- Administer psychopharmacology treatments as needed
- Collaborate with other healthcare professionals to develop comprehensive treatment plans
- Offer PTSD care and support to patients
- Maintain accurate medical documentation and records
- Work with diverse patient populations including pediatrics
- Utilize medical terminology effectively in patient interactions
Requirements
- Must hold a valid license as a LMHP-R, LMHP, OR LPC
- Experience working with children is preferred
- Proficiency in conducting patient assessments and developing treatment plans
- Knowledge of medical documentation practices
- Ability to work in a hospital setting and collaborate with multidisciplinary teams
