Job Description
Seeking an experienced Licensed Master Social Worker (LMSW) for a 13-week Home Health assignment serving patients in Portland, Oregon. The LMSW will provide comprehensive social work services to patients and families in the home setting, helping address psychosocial needs, care coordination, community resources, and barriers to successful home-based care.
Key Responsibilities
- Provide social work assessments and interventions for patients receiving home health services.
- Assess patients' psychosocial, emotional, financial, and environmental needs.
- Develop and implement individualized plans of care in collaboration with the interdisciplinary care team.
- Provide counseling, education, support, and appropriate referrals to patients and families.
- Coordinate community resources and services to support patient safety and independence.
- Assist patients and caregivers with navigating healthcare and social service systems.
- Maintain accurate and timely clinical documentation.
- Complete required OASIS documentation and related home health assessments.
- Coordinate care with physicians, nurses, therapists, case managers, and other members of the care team.
- Travel to patients' homes throughout the assigned service area.
Required Qualifications
- Valid Oregon LMSW license is required.
- Minimum 1 year of skilled home health experience required.
- OASIS experience is required.
- BLS (AHA) certification required.
- Home health/social work clinical experience preferred.
- Strong assessment, communication, documentation, and care coordination skills.
- Ability to work independently in patients' homes.
- Reliable personal transportation is required.
Any Specific Mandate skill required
- Oregon LMSW License – Mandatory
- Minimum 1 year of Skilled Home Health experience – Mandatory
- OASIS experience – Mandatory
- BLS certification – Mandatory
- Home Health Social Work experience – Strongly required
- Ability to conduct independent home visits – Mandatory

Pay: $2,200.00 - $2,400.00 per week
