NORTHEAST DELTA HUMAN SERVICES AUTHORITY MISSION:

Northeast Delta Human Services Authority is seeking a qualified individual to serve as a Social Service Counselor 3 to facilitate the SPOE process of the Integrated Care System across Region 8 Community and hospital treatment setting for all consumer groups. This role will monitor the regional bed allocation process whereby the Region can access its allocation of intermediate care inpatient beds and will also ensure compliance with continuity of care policy. The person in this position is also the primary timekeeper for Monroe Integrated Care Clinic.

Five years of social services experience; OR
Six years of full-time experience in any field plus two years of social services experience; OR
A bachelor's degree plus two years of social services experience; OR
An advanced degree in a social sciences field plus one year of social services experience.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.
NECESSARY SPECIAL REQUIREMENT:
Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.

Job Duties:

Responsible for continuity of care for consumers discharged from private/public facilities as well ensuring that the continuity of care policy is followed by monitoring the continuity of care process, contacting referral sources, and making follow-up calls to agencies, clients, and regional clinicians to maximize continued care.

Schedule patients using the Centralized Scheduling System and provide appointment information to the discharge facility.

Reviews packets for determination of eligibility for aftercare and follow-up by and Integrated Care Clinic in Northeast Delta Human Services Authority's network. This role will complete the Adult Aftercare ICM referral with each packet.

Inspects leave slips and time statements for accuracy and verify hours worked. Runs reports for each pay period to compare sign-in sheets and time statements with reports for accuracy and prepares payroll adjustment forms when indicated.

Updates the daily census for all facilities on Monday.com. This position also handles calls from facilities looking for beds for consumers needing intermediate care, gathers information about the consumer and determine eligibility for facility placement, and contacts facilities to see if bed is available. If a bed is available, the person in this role will provide consumer information, notify the referring agency to arrange transport, and will coordinate the admission of the consumer to involve contact to hospitals, non-profit organizations, physicians, coroners, mental health centers, family members and law enforcement.

Compensation: NEDHSA is a designated approved National Health Service Corps (NHSC) site. Clinicians who work at NHSC-approved sites are eligible to apply to the NHSC Loan Repayment Program to pay off student loans.

2-year Full-Time Clinical Practice: NHSC will pay up to $50,000* for an initial 2 years of full-time clinical practice to clinicians.
(Full-time clinical practice is defined as no less than 40 hours per week, for a minimum of 45 weeks per year.)
