Join our dedicated team as an Experienced Personal Care Assistant, where your compassion and expertise will make a meaningful difference in the lives of those you serve. You will be essential in assisting individuals with daily activities, managing medical needs, and promoting independence within a variety of care settings including assisted living, nursing homes, and home health environments.
Responsibilities
- Provide compassionate assistance with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts.
- Monitor patient health by observing vital signs, recording medical documentation, and reporting changes to healthcare professionals.
- Assist with medication administration following prescribed protocols while maintaining HIPAA compliance and accurate medical records.
- Support patients with dementia, Alzheimer's, memory care, and other cognitive impairments through specialized care techniques.
- Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient interactions accurately and efficiently.
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients.
- Engage in patient observation to identify needs or concerns promptly while providing emotional support and companionship.
- Utilize medical terminology appropriately when documenting or communicating patient information.
- Assist with medical procedures such as catheter care, hospice support, working with individuals with disabilities or developmental challenges.
- Collaborate effectively with healthcare teams to ensure comprehensive patient care within home & community settings or long-term care facilities.
Experience
- Prior experience in senior care, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Demonstrated proficiency in caring for individuals with dementia, Alzheimer's disease, or other memory impairments.
- Hands-on experience working with patients requiring hospice or palliative care is advantageous.
- Knowledge of medical terminology, vital signs monitoring, medication administration, and documentation practices is essential.
- Familiarity with EMR/EHR systems and HIPAA regulations ensures compliant and efficient record keeping.
- Experience operating mobility aids such as Hoyer lifts and performing catheter or hospice care is desirable.
- Strong caregiving skills combined with patience, empathy, and excellent communication abilities to foster trust and comfort among clients.
